| Reverse description | The reverse features a large central shield at right bearing the portrait busts of the three Apollo XII astronauts — Charles Conrad, Richard Gordon, and Alan Bean — with their names inscribed alongside each effigy. To the left, set against a lunar background, are four smaller heraldic shields commemorating the Apollo IX, Apollo X, Apollo XI, and Apollo XII missions, with the respective mission names and crew names of Apollo IX (McDivitt, Scott, Schweickart) and Apollo X (Stafford, Young, Cernan) inscribed on their corresponding shields.
