Obverse description Brown letterpress certificate with ornate guilloche border and scalloped outer frame. The denomination '5 FIVE RUPEES' is printed in large bold type across the upper register, flanked by side panels bearing Devanagari script numerals. The central body carries the full contractual text in English, with fields for the issuing post office, date, register number, and Postmaster signature.

This is to certify that XXXXXXXXXX is registered at the XXXXXXXXX Post Office as the holder of a Post Office 12 Year National Savings Certificate, issued in accordance with the terms of the Notification number printed on reverse, and subject to the Application and Declaration of the Purchaser which shall be the basis of this Contract. The Government of India undertake to pay to him, on presentation of this Certificate at the aforesaid Post Office on or after the XXXXXXXXX or at any earlier date after the expiry of eighteen months the sum not exceeding Rs. 7/8/- specified on the reverse of this Certificate as due on such date.

Reverse description Brown letterpress reverse with matching guilloche border, headed 'RECEIPT ON DISCHARGE.' A ruled two-column table in the left panel lists the redemption values in rupees-annas-paise at successive yearly intervals from 1½ to 12 years, with the minimum holding period noted as '(NOT ENCASHABLE TILL END OF 18 MONTHS)'. The right panel provides spaces for payment acknowledgement, holder signature or thumb impression, and date.
