Blank planchets escape the coining press through mechanical failure or operator error — a mis-fed strip, a timing fault, or an ejection malfunction that pulls an unstruck disc through quality control undetected. Euro cent planchets are produced at private blanking facilities before shipment to national mints, which means the point of escape could precede the actual striking operation entirely, making attribution to any single mint nearly impossible.
