Obverse description The coin is shaped as a ship's helm with eight spokes, framing a circular dark-field medallion. In the central field, a relief depiction of a full-rigged sailing frigate, identified as the USS Constellation (Commodore Truxtun's 1797 frigate), underway on the sea. Five six-pointed stars arc across the upper field above the vessel, representing the five previous ships named Truxtun. The circumferential legend reads around the border of the inner disc.

Reverse description The reverse shares the same ship's helm outline, here with a rope-twist border on the inner disc. The central field features a detailed relief of the Arleigh Burke-class guided-missile destroyer USS Truxtun (DDG-103) underway, with hull number 103 marked on the bow. In the upper left, a color enameled oval ship's crest inscribed USS TRUXTUN / DDG-103 is applied in relief. Above the destroyer, crossed missiles or weapons are depicted in relief in the upper right field.
