| Beschrijving voorzijde | Green underprint with red numerals in the upper left and right corner rosettes, the central vignette rendered in black intaglio presents the crowned Polish White Eagle with wings spread within an elaborate oval cartouche of scrollwork and foliage, surmounted by a royal crown. Columns of text in two flanking panels carry the guarantee statement of the German Empire and the anti-counterfeiting warning, issued under authority of the Board of the General-Governorate of Warsaw and dated 9 December 1916, with two facsimile signatures below the left text block and the year 1917 at the foot of the central vignette. |

| Beschrijving keerzijde | Printed entirely in blue-green, the reverse is dominated by dense, symmetrical guilloche scrollwork and floral ornaments filling the field. Two circular medallion vignettes at upper left and right each contain a female portrait in profile, while two further numeral medallions bearing the digit "5" appear in the centre-left and centre-right. The denomination PIĘĆ MAREK POLSKICH is repeated twice in a lower panel, and the issuer title BILET POLSKIEJ KRAJOWEJ KASY POŻYCZKOWEJ spans the top of the note in bold lettering above a band of fine guilloche. |
